Madison County's high school graduation rate is 83 percent, one percentage point down from in 2016, according to the rankings.
Almost every county nationwide is included in the rankings, which compare the locale to the rest of its state and top performers across the nation.
The County Health Rankings report by the Robert Wood Johnson Foundation and University of Wisconsin Population Health Institute is created to help explain the factors that influence the health of the community. But numerous metrics within that category - including the 17 percent of residents who reported their health as "fair" or "poor" - were relatively stable, suggesting that Guilford County's lower ranking has more to do with improvements in other counties than health deteriorating here.
The ranking, which the foundation performs in collaboration with the University of Wisconsin Population Health Institute, sorts Pennsylvania's 67 counties on two factors: Health outcomes and health factors.
"We already had a pretty good parks and rec system, for instance, but we're continuing to build onto the greenway more", Jackson said.
Wergin noted of the rankings, "Healthiest Manitowoc County uses the annual information used by the County Health Rankings to watch Manitowoc County's health status from year to year, to identify what we can do to improve the health of Manitowoc County and where we are falling short".
Between 2000 and 2015 more than half a million people died from drug overdoses, and the majority (55 percent) occurred from 2009 to 2015.

Ottawa County ranked first in the Health Outcomes category while Washtenaw was ranked first in the Health Factors category.
The percentage of county residents without health insurance also declined from 18 percent to 15 percent. Factors that might have contributed to that slight decline were increases in unemployment, uninsured residents and violent crime.
Florida's healthiest top five counties are: St. John's, Collier, Sarasota, Seminole and Martin. Enroll Wyandotte helped increase the number of residents with insurance and walking clubs have encouraged physical activity, she said.
